ONLINE REGISTRATION for Erasmus incoming students
RESIDENCE PERMIT – documents
- 4 sharp, colour photos – size: 4,5 x 3,5 cm, taken in last 6 months, on monochrome and bright background, clearly showing eyes and face from the top of the head till the upper part of shoulders, the face should occupy 70-80% of the whole photo; the person on the photo should not wear any kind of headdress and dark glasses, should be looking directly with open eyes not covered by hair, with natural expression and closed mouth;
- Passport (page with personal data & photo) – 4 photocopies;
- Page with visa or with stamp confirming crossing the border – 4 photocopies;
- 3 copies of application form-residence permit for a defined period of time (available in International Relations Office);
- Health insurance – original & photocopy;
- Polish sworn translation of insurance card – original & photocopy
- Birth certificate – original & copy;
- Polish sworn translation of birth certificate – original & copy;
- Letter of acceptance or confirmation of GUT’s student status containing duration of studies, exact dates – original & copy (ask at your faculty);
- Document confirming possession of financial assets covering subsistence costs, return travel costs and study fee (traveller’s cheque; confirmation of credit card limit issued by the bank that issued the credit card; account balance confirmed by the bank with stamp and signature; scholarship confirmation issued by university, stamped and signed)*;
- Confirmation of fee payment*;
- Confirmation of temporary domicile – original & 4 photocopies (to collect in the City Hall on 74 Partyzantów Street, needed notification of stay lasting more than 3 months - zgloszenie pobytu czasowego trwajacego ponad 3 miesiace);
- Confirmation of payment 340 PLN (stamp duty): to be payed at the cash-desk in Pomerania Voivodeship Council or by bank transfer;
- Optional authorization - 17 PLN*: to be payed at the cash-desk or by transfer.

SEASON TICKET
Students who have already received their student cards can use them as season tickets for Gdansk municipal transport. To do so, they should fill in the following form: wniosek-karta miejska and submit it along with the student card at one of the branches of ZTM, e.g.:
- Punkt Obsługi Klienta nr 2, 8 Grunwaldzka Avenue (close to the university)
- Punkt Obsługi Klienta nr 3, Main Railway Station in the city centre (tunnel).
After submitting the documents, the student receives a confirmation, that will be valid for 5 days, which will allow him to travel in the municipal transport with student discount.
